Obituary Notice – Jaye D. Borowiak

FORESTVILLE:  Jaye D. Borowiak, 36, of Walnut Street, Forestville, died Thursday, July 27th, 2017, at Roswell Park Cancer Institute, Buffalo, surrendering to Melanoma, after fighting a long hard battle.  He was born in Springville, on March 29, 1981, the son of Lynn M. (Sawyer) Borowiak, of Forestville and the late Daniel L. Borowiak, who died February 18, 2006.

He was a graduate of Forestville Central School, Class of 1999 and worked as a Finish Department Manager for Elite Designs Intl., Ltd Silver Creek, for 15 years.  Mr. Borowiak enjoyed hunting, fishing, gardening and the outdoors.  Survivors include his wife Vanessa M. (Bushman) Borowiak of Forestville, whom he married in August 2009, at Tri-County Country Club, Forestville, a sister, Melissa Borowiak, of Forestville a sister-in-law Lydia (Chris) Brewster of Fredonia, brother-in-law Brett Bushman of Forestville and close family friend Gary Sprague.  Also surviving are his grandmother Diane Sawyer of Cattaraugus, Father-in-law and Mother-in-law Kenneth J. & Mary M. Bushman of Forestville, several aunts, uncles & cousins.

A Celebration of Life will be held at A later date.  Time and place to be announced.  The family suggests memorials may be made either to Roswell Park Alliance Foundation, Roswell Park Cancer Institute, PO Box 631, Buffalo, NY 14240-0631 or to the ASPCA, PO Box 96929, Washington, DC 20090-9629.
